What Kind of Specialist Do You Need?

Irregular or missed periods, unwanted hair growth, stubborn weight gain and trouble conceiving all sit with a gynecologist first. Where the underlying hormone or insulin picture is the bigger problem, an endocrinologist is usually brought in alongside.

An infertility specialist looks at both partners rather than one, orders the semen analysis and ovulation and tubal tests, and then talks through ovulation induction, IUI or IVF — and says plainly when someone else should take over.

An obstetrician carries a pregnancy from the first antenatal visit through to delivery: growth scans, blood pressure and sugar checks, the conversation about how you want to deliver, then labour itself and the weeks after.

An endocrinologist sets the medicine or insulin regimen, chases a sugar level that will not settle, and watches for eye, kidney and nerve complications. Straightforward, stable diabetes is often looked after perfectly well by a medicine specialist.

Take an odd TSH result, an overactive or sluggish thyroid, a visible goitre or a lump in the neck to an endocrinologist. They decide what scanning is warranted and when a surgeon needs to see you.

Bleeding from the back passage, painful piles, a fissure or a discharging fistula belong with a colorectal surgeon. Do not assume bleeding is 'just piles' — it needs looking at, because other causes present the same way.

A dermatologist works out why hair is falling — pattern baldness, alopecia areata, a scalp infection, or something systemic behind it — and treats the scalp condition itself. Blood tests follow if an underlying illness is suspected.

Fatty liver, hepatitis, jaundice, deranged liver enzymes and cirrhosis are a hepatologist's territory. In much of Bangladesh a gastroenterologist covers the same ground and is easier to reach.

Back pain that will not settle, pain shooting down the leg, or a suspected disc problem goes to an orthopedic or physical medicine specialist. Sudden leg weakness or losing control of your bladder is an emergency — go straight to hospital.

An orthopedic specialist sorts out arthritis from a torn ligament or meniscus, handles sports injuries, and investigates a knee that keeps swelling. For rehabilitation without surgery, physical medicine is often the better door.

A urologist decides whether a stone will pass on its own, needs medication, or has to be broken up or removed, and deals with any blockage. Fever alongside severe flank pain must be seen the same day.

A gastroenterologist investigates ongoing abdominal pain, reflux, ulcer symptoms and changed bowel habit. Vomiting blood, passing black stool, or pain that comes on suddenly and severely is an emergency, not a clinic appointment.
